Graduation Transitions Presentations
All grade 12 students are responsible for preparing a Graduation Transitions Presentation as part of the Ministry Requirements for graduation!
Students MUST complete this 4-Credit program in order to graduate. All grade 12 students will make this presentation to their advisory teacher.
Upon successful attainment of the following Prescribed Learning Outcomes, a “Requirement Met” mark will be assigned, and students will earn the four credits required in order to graduate.

Personal Health:
Students must demonstrate the knowledge, attitudes, and positive habits needed to be healthy individuals, responsible for their own physical and emotional well-being, by completing the following:
Task Description |

Documenting 150 minutes a week of moderate to vigorous physical activity. |
|
Giving evidence of physical activity (medals, certificates, photos) |
|
Developing a long-term healthy living plan |
|
Summarizing health goals for future health maintenance |

Community Connections:
Students must demonstrate the skills required to work effectively and safely with others and to succeed as individual and collaborative workers, by completing the following:

Documenting 30 Hours of Work/Volunteer Experience |

Showing evidence to support Work/Volunteer experience (pay stubs, letter, photos) |
|
Completing Young Worker Safety Orientation Checklist |
|
Completing the worksheet – Transferring Skills Acquired to Future Experiences |

Preparing a current Resume |
|
Career and Life:
Students must demonstrate the confidence and competency needed to be self-directed individuals, by completing the following:
Task Description |

Completing “Career Cruising” or information on TWO careers |
|
Completing a response to questions on post-secondary plans |
|
Completing a financial plan for the first year after high school |
